AN ACCIDENT REPORT

1.0  INTRODUCTION

1.1   Definition of accident

Accident is a bad event caused by error or by a chance. Accidents are always unintentional, and they result in some damage or injury.

1.4  Description Of Accident

While employee lift the container using the crane, the cable hit the oil barrel. It’s make the oil spill on the container. When container still hanging, one of the cable is disconnected and cause it not stable. The stone in the pallet fallen into the worker who doing a work below. The crane out of balance its cause the oil spill into the sea and on the ship. One of employee step on the oil and fall.

2.0  FINDINGS

    2.1 Causes of accident

-       The cap of the barrel is not tight and oil spills when it fall.
-       Machinery should be in good condition before starting work.
-       Stones in pallet.

 2.2 Effects on workers / employer

- The worker hit by the stone.

- The worker Fall because step on the oil spill.

- The marine life threatened.

3.0  CONCLUSION

Every employee must follow the standard of procedure that provide by employer. Safety first when doing work. Hence, employer must make sure all employee use the PPE equipment during worked.

4.0   RECOMMENDATION

4.1  The importance of PPE

PPE is equipment that protect workers against health or safety on the job. The purpose is to reduce employee exposure to hazard. The employer must provide the PPE to employee as a preventative measure to minimize the accident to employee. Basic the PPE such as safety helmet, gloves, goggle and safety footwear.

5.0 WHAT IS HAZARD & RISK

-       Hazard is something that can cause harm, e.g. electricity, chemicals, working up a ladder, noise, a keyboard, a bully at work, stress, etc.

-       Risk is the chance, high or low, that any hazard will actually cause somebody harm

  Element Hazard in the video:

-       The machine.

-       The chemical.

Safety Precaution:

-       Focus

Do not disturb the worker when their doing the work and keep alert to the surrounding of workplace
